Certificates/records must contain UTC as the certificate holder, the amount of insurance coverage, the data of the certificate/record where the insurance is valid and an authorized signature. The standard proof of insurance is commonly referred to as „Form E” and is issued by the insurance company. The official name is Uniform Motor Carrier Bodyily Injury and Property Damage Liability Certificate of Insurance. All businesses applying for a UTC permit, certificate or registration must submit and retain proof of liability and property damage insurance. This certificate/workbook is only valid for 60 days. This certificate/workbook must be replaced by Form E before the expiration of these 60 days, otherwise we will cancel the approval.
